Patient empowerment platform and personalised
self-management support mechanisms

ADLIFE Patient Empowerment Platform was developed for involving patients and informal caregivers to care plan management and shared decision-making process. PEP implemented a functionality that allows online PROM questionnaires, in order to continuously collect feedback from the patients about the outcome measures, and to monitor risk factors. Communication Engine was employed to deliver Just-In Time adaptive interventions (JTAIs) to patients in real time.

In the ADLIFE project, patient empowerment is facilitated through a suite of mechanisms each designed for the purpose of making empowerment opportunities available for those patients who seek it. These include sharing care plan of the patient clearly with patients and his/her informal care givers, bidirectional communication via Patient Reported Outcome Measures (PROMs), symptom collection, care plan feedbacks and patient recorded data, reminders and motivational messages to increase adherence to care plan, educational materials and shared decision making tools. Together, these tools aim to empower patients to take charge of their lives and care plans should they aspire to do so. The empowerment mechanisms are delivered via the Patient Empowerment Platform (PEP) which is one of the main components of ADLIFE ICT Toolbox.
In addition to presenting a clear view of care plan responsibilities, ADLIFE PEP supports the patients in their self-management journey, via Just-In-Time Adaptive Interventions (JTAI). JTAIs are used in ADLIFE to remind patients about the activities that are assigned to them as part of their care plan, and to motivate them to reinforce their adherence. The technical architecture implemented enabled the delivery of these personalised adaptive interventions to the patients via PEP. 
The Patient Empowerment Platform has been extended with a number of additional features. Integration with medical devices supporting Bluetooth LE protocol has been enabled to ease data collection from patients. In addition to this, a more user-friendly dashboard has been implemented from scratch to ease care plan review and data collection from patients.
