Patient empowerment platform and personalized
self-management support mechanisms

ADLIFE Patient Empowerment Platform will be developed for involving patients and informal caregivers to care plan management and shared decision-making process. PEP will implement a functionality that allows online PROM questionnaires, in order to continuously collect feedback from the patients about the outcome measures, and to monitor risk factors. Communication Engine will be employed to deliver Just-in time adaptive interventions (JTAIs) to patients real-time.
Objectives
Provide an online environment to the patient for reviewing his/her care plans including the details of the goals and activities
Provide an online platform to the patient to communicate with his/her caregivers regarding his care plan, provide feedback, and outcomes of the activities assigned
Provide an online platform to the patient to provide feedback about their care via Patient Reported Outcome Measures
To present personalized interventions (reminders, warnings, motivations) to the patient in a context aware manner to increase his/her adherence to the care plan

PEP Beta Release implementing the PROMs, Shared decision-making mechanisms, and integrating sensor measurement management by a Mobile app.
