Benefits for the healthcare team

An intelligent digital toolbox to support collaborative, integrated, personalised care

ADLIFE is a research project that aims to support multidisciplinary healthcare teams to work together on shared care plans for patients with advanced chronic conditions, with the assistance of intelligent automated monitoring of each patient's clinical situation.

This not only helps to facilitate timely, integrated, high-quality care, it also contributes to better working conditions and efficient work time management.

How does ADLIFE support integrated care?

Through the ADLIFE platforms healthcare professionals can create and share dynamic, tailored and integrated care plans.

Evaluation of the effectiveness of the ADLIFE toolbox

Throughout this pilot study, we want to evaluate the ADLIFE digital platforms, assess how useful they are to patients and healthcare professionals, and what the effect is on the patients’ quality of life. Below you will find some of the ambitions we will test during the pilot and the ADLIFE features that will facilitate reaching this objective.

The ADLIFE toolbox strives to support healthcare teams to better coordinate care via shared care plans.

What will healthcare professionals experience?

  • Consultation of all relevant clinical patient information in one comprehensive, updated care planning application, integrating patient data from different sources

  • Sharing and coordinating personalised care plans to align treatments and avoid overlap, potential medicine errors and unnecessary hospital visits

  • Communication with care team members through messaging and online meetings

ADLIFE aims to improve working conditions of healthcare providers, optimizing work time management and multi-disciplinary coordination

ADLIFE assists the care team with intelligent tools for clinical decision support.

What automated notifications does ADLIFE include?

  • Early risk detection to anticipate potential problems patients may experience during the course of their illness so that the care team can swiftly react to changing care needs.

  • Timely and personalised evidence-based recommendations for treatments, goals and activities based on validated clinical guidelines and the latest patient context.

ADLIFE’s intelligent assistance is meant to notify healthcare professionals when they have to update the care plan.

ADLIFE facilitates patient involvement and active participation.

What exchange options can improve care?

  • Online communication channels between care professionals and patients facilitate joint decision making and personalised care plans tailored to the patient’s wishes.

  • Health professionals can rely on patient-generated data, such as symptom reporting and measurements, that is fed into the care plan and the clinical decision support platform.

Through patient participation and motivation ADLIFE strives to increase care plan adherence, to protect patient functionality and enhance their autonomy
