Patient Empowerment Platform

The Patient Empowerment Platform (PEP) is the tool that enables patients and their informal caregivers to be better informed of the patient’s care plan and to be actively involved in the management of the disease.

This involves, amongst others:

  • Receive reminders of what to do in daily life, e.g. mediation intake but also goals (e.g. reduce smoking) and activities (e.g. physical exercice)

  • Communicate with the care team and make joint decisions

  • Provide health information so that the care team can better monitor the patient’s health status

  • Have access to educational information on the disease
