Objectives

In WP5, we will develop a Clinical Decision Support System, which aims to facilitate personalization of guideline driven care plans and enhance capacity to respond early and rapidly to individuals' changing needs and preferences over time.

The main functionalities of this system will be:·      

• Provide functional status and needs assessment of the patients and the caregivers

• Offer personalized treatment goals and activities based on clinical guides and most recent context of the patient

• Predict changing clinical status and needs of the patient, improving performance and response on high-priority health conditions

• Provide a secured interface from which the ADLIFE PCPM (Personal Care Management Platform) will receive those risk predictions, needs assessments and personalized treatment goals and activities defined by it.

• Provide a secured interface to take advantage of existing data sets feed by the registered information on EHR (Electronic Health Record) and PROMs (Patient Reported Outcomes Measures) via the TIS (Technical Interoperability Suite) and SIS (Semantic Interoperability Suite) systems.
