Objectives

WP5 developed a Clinical Decision Support System that facilitates personalisation of guideline-driven care plans and enhances capacity to respond early and rapidly to individuals' changing needs and preferences over time.

The main functionalities of this system are:     

• Functional status and needs assessment provision for the patients and caregivers

• Personalised treatment goals and activities based on clinical guides and most recent context of patients

• Predictions of the changing clinical status and needs of the patient, improving performance and response on high-priority health conditions

• A secured interface from which the ADLIFE PCPM (Personal Care Management Platform) receivea those risk predictions, needs assessments and personalised treatment goals and activities defined by it.

• A secured interface to take advantage of existing data sets feed by the registered information on EHR (Electronic Health Record) and PROMs (Patient Reported Outcomes Measures) via the TIS (Technical Interoperability Suite) and SIS (Semantic Interoperability Suite) systems.
