Evaluation of the effectiveness of the ADLIFE toolbox

Throughout this pilot study, we want to evaluate the ADLIFE digital platforms, assess how useful they are to patients, healthcare professionals and health systems. Below you will find some of the ambitions we will test during the pilot and the ADLIFE features that will facilitate reaching this objective.

What will ADLIFE users experience?

  • Primary, secondary and social care professionals and organisations in different settings will have a platform to collaborate on shared careplans for more coordinated multidisciplinary care. Automated decision support will assist in the delivery of safe, timely, personalised care leading to better health outcomes.

  • An increasing number of people with advanced chronic conditions will be triggered to practice more self care and to improve adherence to treatment recommendations, leading to better patient experiences.

ADLIFE helps health professionals working in different care settings to better align treatments, eliminate overlap and avoid unnecessary hospital visits.

The ADLIFE care model aims to raise efficiency and cost-effectiveness, avoiding ineffective usage of resources.

  • Thanks to the ADLIFE platforms, expertise can be deployed where most needed, regardless of location, involving different health professionals to take up the most appropriate roles that fit their profile.

How does the ADLIFE solution work in practice?

Health and care professionals can integrate the Personalised Careplan Management Platform in their daily routine. It is powered by the Clinical Decision Support System.

Patients and their caregivers can rely on the userfriendly Patient Empowerment Platform.
