Talking Head Video 3: ADLIFE Digital Toolbox

Gökçe Banu Laleci Ertürkmen introduces the technological innovation behind the three ADLIFE digital platforms. 

ADLIFE has developed three complementary digital platforms to support elderly patients with chronic conditions, and the health professionals, organisations, and systems that care for them. In this talking head video, Gökçe Banu Laleci Ertürkmen from our consortium partner SRDC (Turkey), introduces the goals and benefits of ADLIFE’s technical Work Packages 3, 4 & 5. 

ADLIFE have developed three innovative digital platforms, consisting of: 

• The Patient Empowerment Platform 

• The Personalised Care Plan Management Platform 

• The Clinical Decision Support Services 

Making up the ADLIFE toolbox, these innovations will enable patients and their informal caregivers to become active participants in the management of their chronic diseases. They can report symptoms and patient reported outcome measures to inform healthcare professionals and care plan development.  

A core objective of the ADLIFE toolbox is to provide integrated care, enabling multidisciplinary care team members across different care settings to create integrated care plans:
