Talking Head Video 1: ADLIFE Pilot Project

In an exciting addition to our talking head series, Dr Rachelle Kaye presents her perspective as the International Projects Coordinator for Assuta Medical Centers (Israel), on the implementation of the ADLIFE pilot project.

“ADLIFE is to me is a really important project because I really think that if you look at the paradigmatic change in the future of health care in all of our systems, integrated care is key.”

Integrated care is a central objective of the ADLIFE research project, with the aim to support multidisciplinary healthcare teams to work together on shared care plans for patients with advanced chronic conditions. This will help achieve communication amongst all individuals and health teams caring for patients, connecting them with the patients, their families, and informal caregivers. Integrated care will ensure that a care plan is tailored to the specific needs of a person, and that it is monitored and updated on an ongoing process.

As Rachelle reflects, this communication is one of the biggest challenges in healthcare systems:

“Very few of us have succeeded in managing true communication and collaboration across the barriers between hospitals and primary care. This is crucial considering the ageing of the population and increasing chronic illnesses. To me this is crucial to the sustainability and the resilience of future healthcare systems.”
