Empowerment of patients, caregivers & communities

WP7 contains the identification, collection and analysis of patient and care professional needs, wishes and requirements for ADLIFE in order to ease the empowerment of both.

The objective of this work package is to enhance patients’ and caregivers’ quality of life and empowerment. The use of the Patient Empowerment Platform is intended to both support and empower the patients and furthermore to enhance the patients and their caregivers’ life quality by:

• Recording the diseases’ effects on the patients and caregivers and explore their needs via Patient Reported Outcomes (PROMs)

• Enhancing ’patients and ’caregivers autonomy by improving individual knowledge and adaptive interventions through the platform with the purpose of enhancing the ’patients and ’caregivers empowerment and adherence to care plans

• Increasing patient ‘centeredness’ allowing shared decision making
