Patient Empowerment Platform & Personalized Self-Management Support

The objectives of WP4 is to deliver the Patient Empowerment Platform as the technical infrastructure to  support patients and their informal care givers in their long term care journey. It will serve the following functionalities:  

• Provide an online environment to the patient for reviewing his/her care plans including the details of the goals and activities  

• Provide an online platform to the patient to communicate with his/her caregivers regarding his care plan, provide feedback, and outcomes of the activities assigned  

• Provide an online platform to the patient to provide feedback about their care via Patient Reported Outcome Measures 

• To present personalized interventions (reminders, warnings, motivations) to the patient in a context aware manner to increase his/her adherence to the care plan
