Region Jämtland Härjedalen

Region Jämtland Härjedalen is sparsely populated and has an aging population. The region is 49341 square kilometers and has a population of 130,000. There are eight primary care areas and about 28 health centres.

Östersunds Hospital has both outpatient and inpatient care (51 outpatient clinics and 416 in-patient beds). Patients with chronic diseases attend outpatient care which collaborates closely with primary care. Primary healthcare is carried out in 28 locations, while home and social care are the responsibilities of its eight municipalities.

Region Jämtland Härjedalen is currently undertaking an organisational-wide initiative moving towards ‘Good and near care’ which focuses on co-ordination, integration, and personalisation of care. This has much in common with the ADLIFE project.

The pilot testing of ADLIFE will take place across the whole Region Jämtland Härjedalen, both at the Östersund Hospital, and also in the primary care units and municipal care where the patients are cared for, and will involve in-patient, out-patient and primary care.
