NHS Lanarkshire

NHS Lanarkshire is a large health board in Scotland, UK serving a population of 655,000 across rural and urban communities. It has 3 acute hospital sites – Monklands University Hospital with 535 inpatient beds, Hairmyres University Hospital with 492 inpatient beds and Wishaw University Hospital with 633 inpatient beds. NHS Lanarkshire primary care services are divided into South Lanarkshire and North Lanarkshire with 6 primary care localities in the North and 4 in the South giving a total of 100 GP practices. Multidisciplinary teams from secondary care services, the health and social care partnership as well as voluntary services work collaboratively to provide the most up-to-date evidence-based care for patients.

NHS Lanarkshire provides both in-patient and community-based treatment as well as social care for patients with Chronic Obstructive Pulmonary Disease and Chronic Heart Disease and their carers. This includes input across a range of services including Cardiology/Respiratory consultant, Heart Failure/Respiratory nurse service, Physiotherapy/Rehabilitation services, Dietician, General Practice Nurse, General Practitioner and District Nurse service, Smoking Cessation, Psychology and third sector/voluntary services.

ADLIFE will aid communication between primary and secondary care teams and provide links between the available services to empower patients and their carers to manage and engage more effectively, be signposted in a timely manner to an appropriate intervention, whether this is to manage their condition, provide prompts, prevent deterioration, or improve understanding.
