Samson Assuta Ashdod University Hospital is located in Ashdod, the fifth largest city in Israel. It is a 300-bed hospital with a potential for expansion to 650 beds and serves half a million residents of Ashdod and the surrounding region. The hospital works in full cooperation with the Ashdod Municipality and the welfare system.

Maccabi Healthcare Services, the owner of ASSUTA, is the second largest health plan in Israel and provides comprehensive primary and secondary community healthcare services to over 2 million people. Maccabi is a key partner in developing the model for integrating hospital and community care in Assuta Ashdod University Hospital and in the ADLIFE project.

The Samson Assuta Ashdod Hospital aspires to be a digital hospital with an overall goal being the development of an integrated care system in collaboration with the Health Plans and the Municipality (social services). This will create interfaces between the hospital and community EMRs predominantly with Maccabi Healthcare Services, and interfaces with Social Services of the Municipality and will provide a fully integrated care system that links the hospital staff, community health care providers, social services, community support services, the patient and the patient's family.

The focus of the ADLIFE in Assuta Ashdod/Maccabi will be the provision of seamless integration between The Samson Assuta Ashdod Hospital, Maccabi community healthcare services, Ashdod municipality social services and other community support resources to support and empower the patients and carers ensuring they receive the care they need in the smoothest and most timely fashion and enhancing both quality of care and quality of life. This will be achieved by creating a cross sectoral (hospital, community, social services) and multidisciplinary team, committed to working together, supported by a digital shared patient care plan platform, monitoring technology and patient empowerment tools.
