The ADLIFE toolbox consists of 3 platforms:

• The Patient Empowerment Platform (PEP) is the tool that enables patients and their informal caregivers to be better informed of the patient’s care plan and to be actively involved in the management of the disease.

• The Personalised Care Plan Management Platform (PCPMP) enables all members of the multidisciplinary care team to manage and coordinate patient-centered, integrated care plans, based on each patient’s baseline and most recent clinical data, following clinical evidence.

• The ADLIFE Clinical Decision Support System (CDSS) helps the healthcare team to personalise the patient’s care plan, based on clinical guidelines, and to respond rapidly to the patient’s changing needs and preferences.
